Bridging Communities provides professional assessments for those 60 and greater to help address the challenges of daily life. Services are based on eligibility for those residing in zip: 48204, 48206, 48208, 48209, 48210, 48216, 48217, and 48238

Access to Services

Bridging Communities leverages the gifts and talents of dedicated staff, volunteers, and partners to fulfill identified clients’ unmet needs. Services range from professional case coordination, friendly phone calls and visits to reduce isolation, safe and supportive transportation to medical appointments, holiday remembrances, supplemental food delivery, access to educational workshops, assistance with Medicare and Medicaid , MI Bridges and MiCafe application assistance along with access to other local resources that support the essentials for daily life.

Service Partners

Partnerships and collaborations with the Detroit Area Agency on Aging, the City of Detroit, and other nonprofit agencies is key to Bridging Communities’ success in servicing the needs of older adults.
